In this role, you will assess, diagnose, intervene with and plan for social work services related to patient and family biological, psychological and social needs, as well as behaviors.
• Assisting patients/families with cognitive disorders (e.g., Alzheimer's disease), movement disorders (e.g. Parkinson's disease) and various types of brain injury (stroke, brain tumor, etc.). • Integrating assessment using information related to cultural, clinical, ethnic, age and other factors into plan of care to implement plans and treatments. Acting as active team member in the Institute for Neurosciences outpatient comprehensive clinic process. • Identifying complicated social and financial factors. Ensuring patient is referred to available and appropriate social and financial resources to achieve the desired outcomes. Referring patients, families and caregivers to resources regarding abuse, neglect, domestic violence and alcohol and substance abuse. • Performing clinical interventions related to grief counseling, bereavement, adjustment and crisis intervention to increase patient and family coping skills.
Requirements: • Master’s degree in social work. • LCSW required. • Three years’ experience in a behavioral health setting. • Licensed Social Worker.
