Health Center of Southeast Texas has locations that are within commuting distance of Humble, Lufkin, Baytown, Houston, and Conroe Texas. Our main clinic is in Cleveland, Texas located approximately 45 minutes north of downtown Houston on Hwy 59. Our other sites are located in Shepherd, Livingston and Liberty.
Health Center of Southeast Texas’ main goal is to address the cultural and financial barrier to behavioral health and healthcare access in Liberty, Polk, San Jacinto and adjacent counties by providing a sliding fee scale and community based primary care services. HCSET is a Federally Qualified Health Center that can provide primary health care services to anyone, regardless of their financial income or insurance status. The clinic was started in 2007 and has grown every year, with further growth projected for the coming years.

Work with children individuals, groups and communities to improve mental health
Oversee and expand the BH program for the company
Run the case management, peer review and quality assurance for the BH program
Report to the Medical director
Encourage clients to discuss emotions and experiences
Examine issues including substance abuse, aging, bullying, anger management, careers, depression, relationships, LGBTQ issues, self-image, stress and suicide
Work with families
Work with local schools and their counseling programs
Help clients define goals, plan action and gain insight
Develop therapeutic processes
Refer clients to psychologists/ Psychiatrist and other services
Take a holistic (mind and body) approach to mental health care
Experience in social work and broad background with diverse patient populations, preferably in medical setting.

Our main goal is to address the cultural and financial barrier to health care access in Liberty county and adjacent areas by providing a sliding scale fee and community based primary care service.
In September 2007, HCSET received Federally Qualified Health Center status which allowed us to grow and expand our services. HCSET is an independent, non-profit 501(c) 3 clinic governed by a board of directors. The clinic is open to all.
