Alluvion Health offers a variety of medical services ranging from Family Medical Care- preventive care as well as acute and chronic illness, physical exams- well child checkups, school/sport physicals, diagnosis and Treatment for Sexually Transmitted Disease (STD), referrals for Specialty Care, and more.
Alluvion Health is dedicated to providing whole person care. Our behavioral health providers work alongside our primary care providers to treat the whole person rather than treating isolated symptoms. Our behavioral health providers provide individual counseling, crisis management, brief therapy for focused concerns, substance abuse therapy and referrals to community resources.
Alluvion Health offers substance use disorder services co-located and integrated with primary care and behavioral health providers. Alluvion Health offers medication assisted treatment (MAT) and licensed addiction counselor services.
Our ideal candidate would encompass a MT license as a clinical social worker and be dually-licensed as an addictions counselor but the LAC is not required.
POSITION AND DUTIES: Provider is employed as a Licensed Clinical Social Worker, Licensed Addiction Counselor.
Time and Duty Requirements: Provider’s services will be provided on a full-time basis pursuant to Alluvion Health policy. Provider’s services will include no less than forty (40) hours of work time per week at Alluvion Health. Provider’s services will include no less than thirty-five (35) clinical hours per week at Alluvion sites in the provision of direct patient care and the remaining five (5) hours per week will be for administrative duties. Provider will participate in Alluvion’s after hours’ coverage pool as designated by the Medical Director. This will include working weekend, holiday and after-hours telephone call coverage on a periodic basis. Frequency of call duties will be determined by the Medical Director or his/her designee, largely based on provider staffing levels.
Alluvion Health's Licensed Clinical Behavioral Health Provider works as a core member of the Collaborative Care Team which involves the patient’s primary care provider and other mental health, addiction or substance abuse providers, as appropriate, to provide comprehensive patient centered health care within the Alluvion Health. The Licensed Clinical Behavioral Health Provider is responsible for coordinating and supporting appropriate patient centered mental health care within the parameters of the Health Center’s Patient Centered Medical Home Certification and ensuring services meet appropriate levels of care based on the patient’s diagnosis and need. The Licensed Clinical Behavioral Health Provider may provide evidence-based treatments or collaborate to provide care with other mental health providers in the Health Center or community when necessary and appropriate.
Provide, support and closely coordinate mental health care with the patient’s primary care provider and, when appropriate, other mental health providers;
Screen and gather information from patients regarding common mental health and substance abuse disorders;Use measurement-based tools to track patients, in person and by telephone, for changes in clinical symptoms and gather information about treatment side effects or complications;
Facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as directed by other team members;
Participate in regularly scheduled caseload consultation with the Psychiatric Consultant and facilitate communication regarding treatment recommendations to the patient’s PCP. Consultations will focus on patients new to treatment or who are not improving as expected, as evidenced by PHQ-9 scores;
Participate in Quality Improvement activities to ensure compliance with Health Center Policies;
Serves as a role model for effective teamwork among staff and actively works to maintain positive morale and attitude;
Safeguards all patients’ health information whether written, verbal, and electronic with privacy and confidentiality. Follow HIPAA Privacy and Security policies and procedures;
Performs examinations, evaluations, assessments, diagnoses, and treatment;
Documents patient histories, progress notes, interventions and orders in the electronic health record;
Serves as a resource person for clinical staff regarding patient care issues
Monitors and improves processes and outcomes of care;
Works collaboratively with Alluvion’s Leadership Team to ensure compliance with HRSA 330 requirements and reporting including UDS, budget period renewals, audits, FFR and any other submissions required for compliance;
Qualifications of Provider:
Be licensed to practice as a Licensed Clinical Social Worker, Licensed Addiction Counselor within the State of Montana; LAC is preferred but not required.
At the request of Alluvion Health, be eligible for credentialing where appropriate for any insurer contracting with Alluvion;
At the request of Alluvion Health, be a member of any Provider-Health Center Organization.
Masters in Social Work or Clinical Mental Health Counseling required; and five (5) years’ experience in a clinical setting preferred.
Must possess a valid driver’s license issued by the State of Montana.
