BHP Licensed Mental Health Professionals consult with Emergency Department Physicians and Nurses in over 30 hospitals across Minnesota and Wisconsin to understand and support people presenting with Mental Health and Substance Use concerns. The current role will focus solely on the M Health Fairview SouthdaleemPATHunit and will partner with nursing and psychiatry to provide assessment,stabilization, intervention, and discharge services to patients with mental health crises.

Clinical Services:
Provides coordinated care byinitiatingclient centered consultation with internal and external providers involved in care or services. Responsible for obtaining the necessary releases
Completes a comprehensive behavioral assessment in person or virtually via telehealth toidentifypatient needs and to recommend a level of care
Providespsychotherapy for clients
Assesses and completes the DEC (Diagnostic Evaluation Center) assessment form for all crisis patients
Determinesappropriate disposition, coordinates care, andprovidesdischarge summaries for patients
Collaborates with the patient, medical doctor, and family to create a treatment plan based on medical necessity and the patients preference for care
Providesprimary communication to collaborating partners,e.g.Hospital emergency departments, schools, primary care clinics, assisted living, and other treatment providers
Adheres to all policies and procedures
Maintains knowledge of available community resources and procedures for hospitalizations
Record Keeping:
Obtains all necessary documents, information, and signatures from patientsin order tocomplete assessments and coordinate care
Conducts risk assessments,de-escalationtechniques/suicide prevention, and/or mental status evaluation
Promptly and accurately documents patient information, the major crisis or treatment issues and expected outcomes based on medical recordkeeping standards. Documents/charts according to department standards,policyand regulatory and accrediting bodies requirements
Service to Internal and External Customers:
Responsible for responding to queriespertaining toservices that can be provided to best meet the needs of the patient
Locates and schedules routine and crisis appointments for patients within the established timeframe
Establishes andmaintainspositive and professional working relationships with internal staff, external service providers and other state and county agencies
Engages inappropriate professionaldevelopment through continuing education, membership in professional organization and licensure as eligible
Asrequested, participates in nonclinical activities such as committee work, quality improvement teams, performance improvement and grant initiatives

Skilled in interdisciplinary teamwork including respectful communication, ability to present a case in a clear format by highlighting discipline-specific and pertinent information, collaborative patient care approaches, and flexibility withcolleagues;
Knowledge of resources in the community and ability to engage in clinical disposition problem solving to connect patients with mostappropriateand least restrictive level ofcare;
Skills in crisis intervention including knowledge of Motivational Interviewing, coping, basicCBTand/or DBT, and brief-solution focused therapy skills;and
Ability to roll with new interventions, structures, and approaches.

Masters Degree, from an accredited school, in Social Work, Psychology, or closely related discipline
Basic Life Support Certification
Valid Minnesota drivers license, access to a car during designated hours, proof of auto insurance coverage
One of the following licenses in good standing in the State of Minnesota:
Licensed Psychologist (Doctoral or Master's)
Licensed Independent Clinical Social Worker
Licensed Marriage Family Therapist (Doctoral or Masters)
Licensed Professional Clinical Counselor (Doctoral or Master's)
Good working knowledge of electronic health records as well as a comfort level and willingness toutilizecomputer-based platforms for assessments along withproficiencywith computers
Ability to handle multiple tasks at one time and to quickly switch from one task to another
Preferred:
Three or more years experience in crisis counseling/situations or clinical therapy
One to three years leadership experience in a behavioral related field, managed care or call center
