Clinical Treatment Coordinator will partner with the Physician and interdisciplinary team to navigate the patient/family through treatment episode, including assessment, treatment planning, discharge planning and community re-entry. This includes, but is not limited to: managing patient/family expectations, responding to the crisis of hospitalization, supporting the discharge and community re-entry needs and monitoring and managing patient length of stay and avoidable days.
Physical Demands: Sitting, standing, walking, push/pull, stooping/crouching, kneeling, grasping, reaching bellow and above shoulders, rotation ,sitting, standing, bending at waist and operate motor vehicle. Talking and and hearing on phone and in person, vision required.
Physical Environment: Noise, heat/cold infectious disease, human tissue/fluids and dirt.
Mental Demands: Frequent people contact, irregular work schedule detailed work, frequent interruption, decision making skills concentration required, work independently, multitask, frequent deadlines, organizations skills and work with others
Qualifications
Required Qualifications:
LISW, LICSW, LGSW, LP, LPC or LMFT licensure
2 years supervised therapy experience in mental health including assessment, individual, group and family therapy
